to be considered for a contributed talk
Payment of Conference Fees: After you register
below, you will be directed to a page for payment. Conference fees can be
paid by credit card or PayPal account. Payment is secure, and DMC will not be
given any of your credit card information. Submitting this form will take you
to a second page where you can choose your options (including student
discounts and additional fees for offsite participants) and pay your fees.
The registration/payment deadline is June 15, 2009. If you cannot register
online or have other questions, please email DMC2009, or Anne
McCoy.
Registration cancellations will result in a full refund if received prior
to June 1, 2009; after that date, the refund will be less a $50
administration fee.
A late fee of $50 is required if registering after June 1, 2009.
Fees:
Registration for Meeting – Includes lunch and dinner,
starting with Sunday dinner and ending with dinner on Thursday as well as
light snacks during the morning coffee break $450 (student and postdoc
registration $400). Breakfast
will not be provided, but all rooms have a refriderator and microwave and
there is restaurant and coffee shop on site. There is also a small store to pick up
breakfast food.
Please indicate the name and numbers of guests as well as any dietary
restrictions on the registration form.
Room reservations - will
be made separately at Snowbird, Room rates are $114 (single) or $124 (double)
plus tax and include and include complementary access to the Cliff Spa; free
in-room high-speed wireless and access to the business center. You should contact Snowbird Resort
